1.  Contact Us.

Call 786-577-9302 or email [email protected]. If we are not available to answer the call, leave a message with your name, your contact information, and the reason for your call. Let us know if you need a specific appointment time. We will get back to you as quickly as possible.

2. Sign up for Patient Portal.

We will send you instructions to access the patient portal. Create your profile and log in. Fill out information regarding your current condition, medical history, medications, and allergies. Make sure to sign electronic copies of your “patient’s rights and responsibilities, privacy notice,  and consent for treatment”. Your information will be kept safe and secure on our HIPAA compliant Electronic Medical Record.

3. Credit Card Information.

Your credit card information will be kept safe and secure along with your medical records. You will not be billed until the conclusion of your treatment.

4. We Will Contact You.

We will contact you by text or email and let you know the approximate arrival time of the physician.

5. Help Is On The Way.

We will meet you at your home or workplace to conduct a general history and physical exam and perform any necessary procedures at that time.

6. Bill.

You will receive an electronic superbill that you can submit to your insurance company for reimbursement.

7. Payment.

Your credit card will be charged at the conclusion of the service.
